UAT Exit Review — Defect Triage & Sign-off Criteria

今日の会議は UAT Exit(UATを終了と認めるか) の判定。

1Priya

This is the UAT exit review. The question on the table is narrow: do we declare UAT complete, yes or no? We are not deciding go-live today — that's next week. Ken, you hold the business sign-off for Japan, so I'd like to hear from you first.

2Ken

Thank you. Let me give you the picture honestly, then my position. We executed ninety-six percent of the test cases. But I want to be transparent about how: attendance was seventy-one percent of plan, and my team made up the gap by working evenings through year-end close.

3Michael

That's not sustainable, and I don't want it normalised. Was that your call or did somebody put pressure on your team?

4Ken

It was my call. I judged that a thin UAT was a bigger risk than a tired team. But I'd like it recorded as a lesson learned, because in Wave 2 we should not schedule UAT over a year-end close.

5Priya

Recorded, and I'll own that mistake — I built that schedule. Let's go to defects. Lena?

Lena。

6Lena

Forty-eight open. Three severity one, eleven severity two, the rest are threes and fours. The exit criteria we agreed say zero severity ones and no more than five severity twos. So on paper, we fail.

7Grace

Then this is a short meeting.

8Lena

It could be — but I don't think a mechanical read of the criteria serves us. Let me walk through the three severity ones, because I'd argue only one of them is genuinely a showstopper.

9Michael

Go on. But be careful — "reclassifying defects" is how projects lie to themselves.

10Lena

Understood, and you're right to be sceptical. First: defect two-one-one. Credit limit checks are not blocking orders for one customer group. That is a real showstopper — we could ship goods to a customer who's over their limit. Fix is in test now, delivery Thursday.

11Lena

Second: defect two-two-four. The Japanese invoice PDF breaks the layout when the customer name exceeds forty characters. Severity one was assigned because it's an invoice, and invoices are statutory. But the impact is cosmetic and it affects nine customers out of two thousand.

12Ken

I need to push back slightly. It's not purely cosmetic — if the registration number gets pushed off the page, the invoice is no longer a valid qualified invoice, and the customer can refuse it. Can you confirm whether the number is still visible?

13Lena

That's a fair catch. I don't know, and I should. Let me verify it today. If the registration number is pushed off, I'll keep it as severity one and fix it before go-live. If it's only the address line wrapping, I'll take it as a severity three with a manual workaround.

14Ken

That works for me. Thank you for checking rather than defending it.

15Lena

Third: defect two-three-one. Batch job for the nightly interface fails if it starts while month-end close is still running. It's a timing collision, not a logic error. The workaround is a documented run order, and honestly the permanent fix is a scheduler change we should make in hypercare, not in the last six weeks.

16Grace

I can accept a documented workaround for that, on one condition: the run order has to be in the operations runbook and the operator has to be trained on it before go-live. Not a wiki page someone bookmarks.

17Priya

Agreed and captured. Now the item I know Ken wants to raise. The OCR accuracy test.

18Ken

Yes. Back in the fit-gap workshop we set a condition: ninety-five percent accuracy, or a manual fallback during hypercare. The result came in at ninety-three point four.

19Michael

So the condition is triggered. Are you invoking it?

20Ken

I am, but let me be reasonable about the scale. Ninety-three point four means roughly one order line in fifteen needs correction. At two hundred lines a day, that's thirteen corrections. One person part-time can absorb that. I'm not asking for a team — I'm asking for two months of one temporary resource and a daily error report.

21Grace

That I can fund. Thank you for coming with a sized ask rather than a concern.

22Michael

Same. Ken, this is the third time you've come to a meeting having already done the arithmetic. It makes decisions fast. I'd like the other country leads to see how you prepare.

23Priya

So — the decision. Lena, restate where we'd be if two-two-four turns out to be cosmetic.

24Lena

One severity one, fixed and delivered Thursday, retested Friday. One severity two accepted with a documented workaround. Nine severity twos remaining against a criterion of five — so we'd still be technically outside the exit criteria on severity twos.

25Priya

Then I'm not going to declare UAT complete today, and I want to be clear that this is not a formality. I'm declaring **conditional exit**: UAT closes on Monday, subject to two-one-one passing retest, two-two-four being confirmed and classified, and the severity twos being reviewed line by line with Ken on Friday.

26Michael

I support that. And to be explicit for the record: if any of those three conditions fails on Monday, I expect to hear it on Monday, not at the go/no-go. Surprises at go/no-go are how you get a No.

27Ken

Understood. And I'd like to state my position formally, since I hold the sign-off. I am prepared to sign off on Monday if those conditions are met. I'm not prepared to sign off today.

28Grace

And for the finance sign-off, I'll add one line: my approval assumes the temporary OCR resource is funded and the runbook is updated. Both are in the plan, so I don't expect a problem.

29Priya

Good. Actions. Lena verifies two-two-four today and reclassifies by end of day. Two-one-one delivered Thursday, retested Friday. Severity two line-by-line review Friday, Ken and Lena. Grace raises the temporary resource request. Ken signs off Monday if conditions are met. Go/no-go is a week from Wednesday.

30Priya

And one more thing before we close. Ken, please pass on to your team that this room knows what they gave up to get us here. I'll say it again at go-live, but they should hear it now.

(Ken [2])

Let me give you the picture honestly, then my position.
We executed 96% of the test cases. But I want to be transparent about how: attendance was 71% of plan, and my team made up the gap by working evenings.

I need to push back slightly. It's not purely cosmetic — [理由].
Can you confirm whether [確認したい事実]?

(Lena [13])

That's a fair catch. I don't know, and I should. Let me verify it today.
If [A], I'll If [B], I'll

I don't know, and I should.(分からない、把握すべきだった)が誠実さの核。

I am, but let me be reasonable about the scale.
That's thirteen corrections a day. One person part-time can absorb that.
I'm not asking for a team — I'm asking for two months of one temporary resource and a daily error report.

(Priya [25])

I'm not going to declare it complete today, and I want to be clear that this is not a formality.
I'm declaring conditional exit: [完了日], subject to [条件1], [条件2], and [条件3].

(Ken [27])

I'd like to state my position formally, since I hold the sign-off.
I am prepared to sign off on Monday if those conditions are met. I'm not prepared to sign off today.
